With every follow-up order, you fill in an evaluation form. This is a short medical check the doctor uses to assess how your treatment is going and whether your dosage is still appropriate. Without a completed form, the doctor cannot issue your prescription and your next order won't be processed.
When does the form appear?
The evaluation form appears automatically in your portal once payment for your follow-up order has been received. You'll also receive an email reminder.
Where do I find the form?
Log in to your personal account. If the form is ready, you'll see a banner at the top of your treatment page. Click it to open and fill in the form.
What's in the form?
The form contains a few questions about:
Your weight and any weight loss
Any side effects you've experienced
Your general wellbeing
Filling it in takes 2–3 minutes on average.
Fill it in quickly = your order moves faster
The sooner you fill in the form after payment, the sooner the doctor can review your prescription and ask any extra questions. So preferably fill it in right after receiving the payment confirmation.
